a. Scope of Works
The contractor shall provide Medical, Life, and Disablement Insurance coverage for all personnel. The scope of work is as follows:
a. Medical Health (Hospitalization & Surgery) Insurance for NPCT1 employee and their eligible dependents
b. Life Insurance for NPCT1's employee
c. Accident Death and Disablement (ADD) for NPCT1s employee; and
d. Total and Permanent Disability (TPD) for NPCT1s employees
e. Warrant that the medical insurance provided is applicable in Indonesia and overseas, at least in Singapore and Malaysia as well 24 hours medical hotline service is available.
f. Membership card valid either in Indonesia or overseas, at least in Singapore and Malaysia.
g. Cashless system for medical service in Indonesia and overseas, at least in Singapore and Malaysia.
h. Report of in and out participant base on updating from the NPCT1.
i. Coordination with the hospital for smoothness of administration/registration of hospitalization, claim, etc.
Details of coverage insurances are detailed in Section B and the related appendices to this tender document.
b. Eligibility
Open for vendor with following requirement:
Have a minimum of 3 years experience in related business, supported by list of local and overseas clients and any achievement/track records in similar project within the last 3 years; and
a. Have proven membership in Asosiasi Asuransi Jiwa Indonesia (Indonesian Life Insurance Association)
b. Have membership card is valid either in Indonesia or overseas, at least in Singapore and Malaysia.
c. Have number of participants, corporate and individual, classified per relevant industry (must be >3000 participants).
d. Have license to provide product of Health Insurance issued by Financial Services Authority of Indonesia (OJK)
e. Average of Risk Based Capital (RBC) is minimum 250% based on 71 /POJK.05/2016 for the last 3 years
f. Provide statement letter warrant that have provider hospital in overseas, at least in Singapore and Malaysia are cash less
